We hope that all CMS students will consider getting involved in extracurricular activities. It is a great way to make new friends and to discover new hobbies.

Students trying out for or playing a 2024-2025 sport, must have an updated physical (completed on/after 4/1/24) on file in the CMS office. CMS will no longer accept paper copies of student physicals.  Physicals must be submitted through Final Forms.  For more information on how to create your Final Forms account, visit the "Athletic Documents" tab.

A new physical is required to be completed after April 1, 2024 to participate in summer athletic activities and all other activities like tryouts, open gyms, open fields, practice, etc. This new physical will be good through the 2024-2025 school year.

CMS offers sports passes that are good for admission to events at Carroll Middle School.  Passes are not valid for tournament games (conference championship games at the end of each season).  Please fill out this form and bring it (along with payment of cash or check) to the Carroll Middle School office
