SPORTS PHYSICALS - Attention Athletes....Due to unusual circumstances presented with the COVID-19 crisis, the IHSAA deemed 2019-2020 Physicals to be valid & acceptable for the 2020-2021 school year for both middle & high school students.  Students that did not have a physical for the 2019-20 school year & wish to participate in athletics for the upcoming 2020-21 school year, will be required to get a physical. 8th Grade Physicals on file at CMS will be forwarded to the CHS Athletic office. 
 

DEVICE COLLECTION PLAN FOR Middle School:

• Grades 6, 7 - Unless you expect to withdraw from NACS this summer, please store the Chromebook, adapter, and case for the summer and have your child bring them back when school resumes in August. If repairs are needed, please turn in the damaged item with a note describing the issue so it can be repaired over the summer. If your child will not be attending an NACS school next year, please turn in the device and adapter, making sure the barcode and/or child’s name are still affixed. Attach a note to explain why it is being turned in. Turning in the case is optional. We will take those still in good condition or you may keep it.
• Grade 8 - Unless your child is planning to attend a summer school class, we need to collect the Chromebook and power adapter. Please confirm that the barcode stickers and/or student’s name stickers are still attached to the Chromebook and adapter. If both are missing, please tape a note with the child’s name to the item. Turning in the case is optional. We will take those in good condition or you may keep it. If your child is enrolled in a summer school class at Carroll HS, please keep all items for use during the class. Items will be collected at the end of the summer session.
