New Student Information


We ask that students who are interested in taking an accelerated course provide their most recent report card, most recent ISTEP/ILEARN or equivalent state standardized test scores, and NWEA record (if applicable). The qualifications required to be placed into our accelerated options are outlined on grade level course selection sheets. We will be offering NWEA testing on the following dates for students who are interested in accelerated but have not taken the assessment before. Please call our school office @ 260-637-5159 to register for one of these dates.

2020-2021 New Student Tour Schedule:

Tuesday, June 9th @ 1:00 PM
Tuesday, June 23rd @ 9:00 AM with NWEA following the tour
Wednesday, July 22nd @ 9:00 with NWEA following the tour
Tuesday, July 28th @1:00

If new students are unable to make it a tour date noted above, then they are welcome to attend a guided tour at registration. 
Registration Tours:
Monday, August 3rd @ 12:00, 2:00, 4:00, or 6:00
Tuesday, August 4th @ 7:30, 9:30, 11:30, or 1:30 

The following paperwork must be filled out and returned to enroll your student at Carroll Middle School. Please note that a verification of residence, a copy of a parent's driver's license, and a copy of your student's birth certificate are requested to enroll your student at CMS.

The following are acceptable proof of residency requirements:

Current Month Utility Statement; gas, water, or electric only-no phone or cable bills
*Driver's license must match the address on the utility bill

Signed Rental Agreement with dates of residency,
*Driver's license must match the address on the Rental Agreement

Closing statement from purchase of home
*Driver's license must match the address on the Closing Statement

If you are building a new home, you will need a Move-In Affidavit from our Superintendent's office. See verification of residency form for additional information.

If you are residing with a 3rd party who lives in the district, you will need a Live-With Affidavit from our Superintendent's office. See verification of residency form for additional information.

If at all possible, please complete the online enrollment information and bring in any necessary enrollment documentation to CMS prior to school registration on August 3rd and 4th. You can access new student registration and enrollment information using the links provided below.

You may bring this enrollment information into Carroll Middle School from 8am to 3pm, Monday through Friday. Additionally, you may fax the completed forms to 260-637-5478, attention: guidance counselors.

Upon receiving the required paperwork, a guidance counselor will enroll the student and create a student schedule. During the school year, this process takes 1-2 school days. If you enroll over the summer, the process will be completed based upon the guidance department summer schedule. If you have questions about the summer schedule, please call 637-5159. Once a student is enrolled in our computer system, a guidance counselor will set up a time to give the parent and student a tour of Carroll Middle School. If enrolling over the summer, please see the dates listed above for planned tours and call our office to schedule for one of those options, if possible. New student paperwork can also be picked up during any scheduled summer tour.
